Patient Checklist (What Should I Bring to the Health Center?) First Visit: 1. If you have insurance, bring your insurance card, list of current medications and co-pay (if applicable) with you to the health center. If you do not have insurance, bring $15, proof of income from the last 30 days (if available), and a list of current medications that you are taking with you the health center. The SHHC offers care for children of all ages, but everyone under 18 must have the consent of a parent or guardian.
