Patient Checklist (What Should I Bring to the Health Center?) First Visit: 1. If you have insurance, bring your insurance card, list of current medications and co-pay (if applicable) with you to the health center. If you do not have insurance, bring $15, proof of income from the last 30 days (if available), and a list of current medications that you are taking with you the health center. All patients who do not have insurance will have their fees determined by our sliding fee scale. To learn more about the sliding fee scale and our payment policies, please click here. The SHHC offers care for children of all ages, but everyone under 18 must have the consent of a parent or guardian. To learn about exceptions to this policy regarding patient privacy, please call the health center at 412-422-SHHC. New Patient Forms: Please click here if you did not receive the new patient paperwork in the mail and would like to print it out before your first appointment. You may type your responses into the 'Patient Demographics and Registration' and 'Consent to Treatment' forms if you prefer, but you will need to sign and date them by hand.
