Patient Checklist (What Should I Bring to the Health Center?) First Visit: 1. If you have insurance, bring your insurance card, list of current medications and co-pay (if applicable) with you to the health center. If you do not have insurance, bring $15 and a list of current medications that you are taking with you the health center. All patients who do not have insurance will have their fees determined by our sliding fee scale. To learn more about the sliding fee scale and our payment policies, please click here. The SHHC offers care for children of all ages, but everyone under 18 must have the consent of a parent or guardian.
