![]() ![]() | Am I Eligible for Care at the Health Center? Yes. Everyone is welcome at the Squirrel Hill Health Center. The Center is open to everyone who seeks top quality primary care. As a federally qualified health center, SHHC serves those who are deemed to be "medically underserved" or disadvantaged when it comes to acquiring care. We accept all major forms of private health insurance, Medicare, Medical Assistance and have an income based sliding fee scale for those without insurance. The health center does not discriminate based on religious beliefs, race, gender, ethnicity, age, physical disability, or ability to pay. We have made it our special mission to embrace and serve all people. The SHHC offers care for children of all ages, but everyone under 18 must have the consent of a parent or guardian. What do I need to do in order to become a new patient at SHHC? 1) Records Request: If you are already an established patient at another medical provider's office, please submit a records request to your former primary care physician (PCP) in order for our office to receive your records by your first appointment. You can click here to download that form or you can request the form from our front office staff. 2. Switch your PCP on your insurance card: If you have health insurance that requires you to select a primary care physician (many Medical Assistance programs and HMO's require patients to select a PCP that will be listed on their insurance card), you must change your PCP to a physician at Squirrel Hill Health Center prior to your first visit. This change can usually be made by calling the member services phone number listed on your card.
